Please
note that as the provider, I am required to do daily health checks on your
child. A daily health check occurs when the child arrives at the program and
throughout the day. This is to insure that your child is in good health to
attend The Boutchkies Family Day Care.
I
will check the childs behavior and appearance:
Childs behavior: is
it typical or atypical for the time of day and circumstance?
Childs appearance:
Skin: pale, flushed,
rash
Eyes, nose and
mouth: are they dry or is there discharge? Is child rubbing their eyes,
nose and/or mouth?
Breathing: Normal or different
(cough)
I
will ask the parent/guardian:
How did your child
sleep?
Eating/drinking
normally? When was the last time he/she ate or drank?
Any unusual events?
When was the last
time your child used the toilet or was changed? Are the bowels and urine
normal?
Has your child
received any medication or treatment?
In
addition, I will check:
Any evidence of
illness or injury since the child was last at The Boutchkies Family Day Care.
Any indication of
maltreatment and child abuse.

note that as a child care provider, the NY State Social Service Law
413: considers me to be a Mandated Reporter - "Persons and officials
mandated by law to report cases of suspected child abuse or
maltreatment."
